Routine bed rest after embryo transfer is not generally recommended, and evidence reviewed by the American Society for Reproductive Medicine (ASRM) has not shown that bed rest improves IVF pregnancy or live-birth rates. A gestational surrogate should follow the specific instructions from her fertility clinic, especially if her medical team gives individualized restrictions.<\/p>\n<h2>Why was bed rest recommended after embryo transfer in the past?<\/h2>\n<p style=\"text-align:left;\">Bed rest was historically used because of concern that standing, walking, or normal movement immediately after transfer might interfere with implantation or somehow cause the embryo to leave the uterus. That idea can sound intuitive, especially after so much preparation has gone into the transfer.<\/p>\n<p style=\"text-align:left;\">However, an embryo placed inside the uterine cavity does not simply fall out when someone stands up. Research comparing different periods of rest with getting up shortly after transfer has not demonstrated a benefit from routine bed rest.<\/p>\n<h2>What does the evidence say about bed rest after embryo transfer?<\/h2>\n<p style=\"text-align:left;\">ASRM&#8217;s guideline on performing embryo transfer reviewed randomized trials, systematic reviews, cohort studies and other evidence on bed rest after transfer. It concluded that there is good evidence not to recommend bed rest after embryo transfer.<\/p>\n<p style=\"text-align:left;\">Importantly, that does not mean every surrogate should ignore her clinic&#8217;s instructions. Fertility clinics may have their own post-transfer protocols, and an individual patient may have medical circumstances that call for different advice.<\/p>\n<h2>Can a surrogate walk around after embryo transfer?<\/h2>\n<p style=\"text-align:left;\">In an uncomplicated transfer, ordinary gentle movement is generally not considered a reason for the embryo to fail to implant. Walking to the bathroom, getting into a car, moving around the house, or doing other normal low-intensity activities does not need to be treated as if it could make the embryo fall out.<\/p>\n<p style=\"text-align:left;\">The fertility clinic should still provide clear instructions about exercise, lifting, work, travel, sexual activity, medications and any other restrictions during the post-transfer period. Those instructions should take priority over general information online.<\/p>\n<h2>Should a surrogate avoid exercise after transfer?<\/h2>\n<p style=\"text-align:left;\">Bed rest and exercise restrictions are not exactly the same question. A clinic may allow normal daily movement while asking the surrogate to temporarily avoid strenuous exercise, heavy lifting, high-impact activity, overheating, or other activities based on its protocol and her individual circumstances.<\/p>\n<p style=\"text-align:left;\">ASRM guidance for gestational-carrier arrangements specifically recognizes that expectations about exercise and other lifestyle choices should be discussed between the carrier, intended parents and care team. The surrogate remains the person who gives consent for her own medical care.<\/p>\n<h2>What should intended parents do after the transfer?<\/h2>\n<p style=\"text-align:left;\">The waiting period after embryo transfer can be emotionally intense, and intended parents may understandably want to control anything that could affect the result. The most helpful approach is usually to support the surrogate in following her clinic&#8217;s instructions rather than adding extra restrictions that have not been medically recommended.<\/p>\n<p style=\"text-align:left;\">It is also important not to interpret normal movement, a busy day, or a lack of symptoms as evidence that the transfer has succeeded or failed. Pregnancy is confirmed according to the fertility clinic&#8217;s testing schedule, usually with a beta-hCG blood test.<\/p>\n<h2>When should a surrogate contact the clinic?<\/h2>\n<p style=\"text-align:left;\">The clinic should tell the surrogate which symptoms require a call and how to reach the medical team after transfer. She should contact her clinic promptly if she develops symptoms the clinic has identified as concerning, or whenever she is unsure whether a medication, activity, illness or symptom requires medical guidance.<\/p>\n<p style=\"text-align:left;\">If the clinic gives individualized instructions because of a specific medical issue, those instructions are more relevant than general embryo-transfer guidance.<\/p>\n<h2>What is the bottom line?<\/h2>\n<p style=\"text-align:left;\">For most uncomplicated embryo transfers, routine bed rest is not supported by evidence as a way to improve success.
