Last reviewed: August 2026.<\/strong> Being Rh-negative does not automatically prevent someone from becoming a gestational carrier. What matters medically is whether an Rh-negative carrier could be exposed to Rh-positive fetal blood and whether she has already developed relevant antibodies. Blood typing and antibody screening allow the medical team to assess this.<\/p>\n

What does Rh-negative mean?<\/h2>\n

The Rh factor is a protein found on red blood cells. A person whose red blood cells have the protein is Rh-positive; someone without it is Rh-negative. Rh status is inherited.<\/p>\n

Why can Rh status matter during pregnancy?<\/h2>\n

If a pregnant person is Rh-negative and the fetus is Rh-positive, fetal red blood cells can sometimes enter the pregnant person\u2019s circulation. The immune system may then produce antibodies against Rh-positive cells, a process called Rh alloimmunization or sensitization. Those antibodies can affect an Rh-positive fetus in a current or future pregnancy.<\/p>\n

How are Rh problems prevented?<\/h2>\n

For an Rh-negative pregnant person who has not already developed Rh antibodies, clinicians may use Rh(D) immune globulin (RhIg). RhoGAM is one brand of RhIg, but it is not the only product. ACOG describes routine prophylaxis at around 28 weeks and, when indicated, within 72 hours after delivery of an Rh-positive baby. RhIg may also be indicated after certain bleeding events, procedures or abdominal trauma.<\/p>\n

What if the surrogate already has Rh antibodies?<\/h2>\n

RhIg prevents antibody formation; it does not treat someone who is already sensitized. If clinically significant antibodies are present, the pregnancy may require additional assessment and monitoring. Whether a sensitized candidate is appropriate to serve as a gestational carrier is a medical decision for the fertility clinic and relevant specialists, not a universal agency rule.<\/p>\n

Does the surrogate need the same blood type as the intended parents?<\/h2>\n

No. A gestational carrier does not need to match the intended parents\u2019 ABO blood type. Rh status is managed as a pregnancy-care issue. If the genetic contributors make an Rh-positive fetus possible, the carrier\u2019s clinician can determine the appropriate testing and preventive care.<\/p>\n

What if the sperm source is Rh-negative?<\/h2>\n

If the relevant genetic circumstances establish that the fetus will be Rh-negative, routine RhIg prophylaxis may not be necessary. ACOG notes, for example, that when the sperm source is known to be Rh(D)-negative, antenatal prophylaxis can be unnecessary in appropriate circumstances. The treating clinician should make that determination.<\/p>\n

What should an Rh-negative surrogate expect?<\/h2>\n
  • Blood typing and an antibody screen<\/li>
  • Review of prior pregnancies, transfusions and relevant medical history<\/li>
  • Repeat antibody testing when clinically indicated<\/li>
  • RhIg at recommended times if she is unsensitized and prophylaxis is indicated<\/li>
  • Additional specialist monitoring if clinically significant antibodies are present<\/li><\/ul>\n

    Bottom line<\/h2>\n

    Rh-negative status by itself is not a reason to assume someone cannot be a surrogate. Modern obstetric care has effective strategies to reduce Rh alloimmunization in unsensitized Rh-negative pregnancies. Eligibility should be based on the candidate\u2019s antibody status, medical history and the fertility clinic\u2019s individualized assessment.<\/p>\n
