Their purpose is not to guarantee a conflict-free or emotionally easy experience. Instead, they help the gestational carrier, intended parents and professionals explore readiness, expectations, communication, support and informed consent before treatment begins.

Why psychological evaluation matters

ASRM recommends psychosocial evaluation and counseling for gestational carriers. Surrogacy involves medical treatment, pregnancy, relationships with intended parents, family responsibilities and potentially difficult decisions. A qualified mental-health professional can help identify issues that deserve discussion before the parties commit to a match.

What may be discussed?

Motivation for becoming a gestational carrier

Past and current mental-health history

Prior pregnancy and postpartum experiences

Support from a partner, family or other important people

Communication style and boundaries with intended parents

Expectations about appointments, pregnancy updates, birth and postpartum contact

Views on prenatal testing, pregnancy termination and multifetal pregnancy reduction

Stress management and practical responsibilities such as childcare or work

Expectations about compensation and the relationship after delivery

The exact process varies by program and professional. It may include interviews, questionnaires or standardized psychological measures, but no single test determines whether someone will have a successful journey.

Is it a pass-or-fail test?

Psychological evaluation should not be treated as a performance or a test that a candidate needs to “win.” The goal is accurate disclosure and informed preparation. Sometimes the evaluation may identify a concern that warrants additional counseling, clarification, delay or a recommendation not to proceed. That is part of the screening function, not a judgment of someone’s worth.

What about the intended parents?

Psychological consultation can also help intended parents consider expectations, communication, grief or prior fertility experiences, boundaries, disclosure to future children and how they may respond to difficult pregnancy scenarios. A strong match depends on more than the carrier’s readiness alone.

Does screening prevent conflict?

No screening process can guarantee that misunderstandings, stress or emotional difficulties will not occur. Good psychological preparation may help identify mismatched expectations early and create a framework for communication, but relationships and pregnancies remain unpredictable.

Medical decision-making remains with the gestational carrier

ASRM emphasizes that the gestational carrier is the person who gives consent for her own medical care. Psychological counseling and legal agreements can help the parties discuss difficult scenarios in advance, but they do not transfer medical decision-making authority to intended parents.

Ongoing support can matter too

The need for emotional support does not end after initial screening. Programs may offer or recommend counseling during pregnancy and postpartum, particularly after pregnancy loss, medical complications, relationship strain or other difficult events. Ask in advance what support is available, who provides it and who is responsible for the cost.

How to prepare for the evaluation

Be accurate about your mental-health, pregnancy and family history.

Think about what level of communication you want with intended parents.

Discuss the journey with the people who will form your practical support system.

Consider how you feel about difficult medical and pregnancy scenarios.

Ask what information from the evaluation will be shared and with whom.

Bottom line

Psychological evaluation is one safeguard within a larger process that also includes medical screening, independent legal counsel, insurance and financial planning, and fertility-clinic clearance. It can improve preparation and help surface important issues early, but it cannot guarantee a smooth or successful surrogacy journey.

This article provides general educational information and is not medical or mental-health advice.

Psychological evaluation in gestational surrogacy helps explore readiness, support, communication, boundaries and difficult pregnancy scenarios.
