There is no single nationwide agency checklist, and agency prescreening is not the same as final medical clearance by the fertility clinic.<\/p>

1. Initial agency conversation<\/h2>

The agency may ask about your age, location, pregnancy history, previous deliveries, current health, medications, support system and reasons for considering surrogacy. This is also your opportunity to evaluate the agency and ask about compensation, insurance, legal representation, matching and support.<\/p>

2. Detailed application and authorizations<\/h2>

A detailed application may cover medical, obstetric, social and household information. Be accurate about prior pregnancy complications, surgeries, medications and mental-health history. You may be asked to sign authorizations allowing relevant medical records to be obtained securely.<\/p>

3. Review of pregnancy and medical records<\/h2>

Records from previous pregnancies and deliveries help professionals identify issues that may affect a future pregnancy. Record collection time varies by healthcare system and provider; there is no universal six-week timeline.<\/p>

4. Psychological evaluation and education<\/h2>

A qualified mental-health professional with third-party reproduction experience may explore motivation, expectations, family support, boundaries, communication, pregnancy termination or selective-reduction discussions, relationship stress and postpartum considerations. The goal is informed preparation and identification of concerns\u2014not simply a pass\/fail judgment about whether someone is \u201cmentally capable.\u201d<\/p>

5. Background and practical-support review<\/h2>

Programs may conduct background checks and review practical support. A home visit is not universal. The purpose should not be to decide whether you have an environment suitable to \u201craise someone else’s child\u201d; a gestational carrier is carrying a pregnancy for intended parents, not applying to parent their child.<\/p>

6. Matching with intended parents<\/h2>

If agency prescreening is satisfactory, you may review intended-parent profiles and participate in a match call. Matching should be mutual: you should be free to decline a proposed match without being pressured to proceed.<\/p>

7. Fertility-clinic medical clearance<\/h2>

The treating fertility clinic performs its own evaluation before embryo transfer. This can include review of obstetric records, physical examination, laboratory testing and assessment of the uterus according to the clinic’s protocol. A candidate accepted by an agency can still be declined by the clinic.<\/p>

8. Independent legal contracts and insurance review<\/h2>

Before treatment proceeds, the gestational carrier and intended parents generally negotiate a written agreement with independent reproductive-law counsel. Insurance should also be reviewed for maternity coverage, surrogacy exclusions and potential gaps. The sequence varies by program and state.<\/p>
